If you are seeking care for more than one child, please fill out a separate form for each child.

Please note that requesting a slot is NOT a guarantee that a slot is available, and if a slot is offered, we have a separate enrollment process. If a slot is not currently available, completing this form places you on the waitlist. We prioritize current staff and then families currently enrolled in our program. Thereafter, slots are offered in the order in which they are received.
